Sleep — you'll spend a third of your life in its blissful embrace. But it isn't a luxury: you need it as much as nutrition and exercise. An occasional bad night's sleep leaves you irritable and drowsy; a bad night's sleep every night could endanger your health.
One of the most common causes for chronic poor sleep is obstructive sleep apnea. This occurs when the airway becomes blocked and you stop breathing temporarily. The blockage may be due to an oversized tongue, tonsils or uvula, an abnormal jaw or chin structure, or nasal polyps and congestion. When your brain notices you're not breathing, it rouses you just enough to relieve the blockage. These incidents can occur and end in seconds several times a night without you being aware of it.
This interrupts your normal sleep patterns, including the critical rapid eye movement (REM) of deep sleep that occurs at different times during the night. The results of not getting enough REM sleep are quite unhealthy: besides irritability and reduced concentration, poor REM sleep is linked to depression, headaches, decreased sex drive, acid reflux, high blood pressure or the onset of diabetes. Your night time experience — as well as your sleep partner's — won't be pleasant either as you may experience night time sweating and snoring.
Fortunately, sleep apnea can be treated. Our first considered treatment is a custom-fitted night guard you wear while you sleep that holds the tongue back from the airway. If your apnea is more severe, you may need to consider continuous positive airway pressure (CPAP) therapy, which uses a machine to pump pressurized air through a mask you wear while sleeping to force the airway open. You might also benefit from surgery to remove excess soft tissue obstructing the airway.

If your sleeping partner snores, it could be more than an annoyance: it could be a sign of sleep apnea. This occurs when air flow into the lungs becomes obstructed in the throat for a few seconds during sleep. The obstruction can take many forms, but a common one arises from the tongue relaxing against the back of the throat, producing snoring sounds as air attempts to pass through this restricted area.
Sleep apnea can cause severe problems: lower daily energy levels and mood from poor sleep; lower oxygen saturation that could affect brain function; and increased risk for cardiovascular disease. So, if you’re awakened by your partner’s snoring (or they’re complaining about yours!), it’s important to have it checked and treated.
This begins with a visit to us for a complete oral examination. Like many dentists, we’re well trained in the anatomy and structures of the mouth, as well as the causes and treatment of sleep apnea. We’ll examine your mouth, take into account any possible symptoms you’re experiencing and, if your suspicions are correct, refer you to a sleep physician to diagnose if you have sleep apnea.
Treatment will depend on its cause and severity. An oral appliance worn during sleep is the recommended first treatment for mild to moderate sleep apnea that involves the tongue as an obstruction. We develop a custom appliance that helps move your tongue away from the back of the throat, reducing both apnea and snoring sounds. For more advanced sleep apnea you could benefit from a Continuous Positive Airway Pressure (CPAP) machine. This device generates continuous air pressure through a mask worn while sleeping that helps keep the airway open.
Of course, there are other causes for obstruction, some of which may require surgical intervention to relieve the problem. Abnormally large tonsils, adenoids or excessive soft tissue can all restrict air flow. Surgically removing or altering these structures could help reduce airway restriction.

Daily fatigue or complaints of your snoring from family have led you to see your doctor about the problem. After an exam and a test session in a sleep lab, your problem now has a name — obstructive sleep apnea.
This common sleep-related breathing disorder (SRBD) occurs when the soft tissues in the back of the throat over-relax during sleep. The relaxed tissues obstruct air flow to the trachea (windpipe) and cause “apnea,” where you cease to breathe. The lack of oxygen causes you to wake, even for a micro-second, to begin breathing again. This may occur multiple times throughout the night, diminishing the quality of your sleep and leading not only to drowsiness and daily fatigue but also contribute to cardiovascular disease or other systemic conditions.
The most effective treatment for sleep apnea is the use of a Continuous Positive Airway Pressure (CPAP) machine while you sleep. The machine delivers pressurized air to a face mask you wear while you sleep; the additional pressure keeps the airway open. However, a CPAP machine does have a few disadvantages, including discomfort while attached to the machine, nasal congestion and dryness, or claustrophobia. These effects can be so pronounced for some patients, they’re unable to adjust themselves to the machine.
If you have mild to moderate sleep apnea, there may be an alternative — a custom-fitted oral appliance we manufacture for you to wear in your mouth while you sleep. The appliance pulls the lower jaw forward resulting in a wider opening of the airway. In addition to being less cumbersome than a CPAP mask, an oral appliance is easier to wear, compact in size for easy travel and doesn’t require electricity.
While an oral appliance is an effective alternative to a CPAP machine for many patients, it does have a few disadvantages including problems with saliva flow (too much or too little), muscle or teeth soreness and minor tooth or jaw movement. Still, an oral appliance might be the right solution to relieve your sleep apnea over the long-term.

Sleep apnea, a form of sleep-related breathing disorders that is estimated to affect some 22 million Americans, is sometimes thought of as the “quiet culprit” lurking behind many other maladies. But ask anyone who sleeps alongside a sufferer, and you'll get a different response: It isn't quiet at all! Instead, it's often marked by loud snoring and scary episodes where breathing seems to stop. If you've ever worried that you or someone you care about may have this condition, here are five facts you should know.
1) Sleep apnea is a potentially deadly disease
For one thing, it leads to chronic fatigue that can make accidents far more likely — a special concern in potentially dangerous situations, like operating machinery or driving a vehicle. It also appears to be related to heart conditions such as high blood pressure, irregular heartbeat, coronary artery disease, and even stroke. Plus, it can lead to weight gain, depression and mood disorders.
2) People with sleep apnea may wake hundreds of times every night
These “micro-arousals” may occur 50 or more times per hour, and may keep a person from getting any relaxing sleep — even though they retain no memory of the episodes. That's why people who suffer from sleep apnea often go through their days on the verge of exhaustion. And they aren't the only ones who suffer: Their bed partners may also be kept up throughout the night, becoming anxious and irritable.
3) Persistent snoring can be a symptom of sleep apnea
Snoring is caused when breath being drawn into the lungs is obstructed by soft tissue structures in the upper airway. Most everyone snores sometimes… but chronic loud snoring is a common symptom of obstructive sleep apnea (OSA) — and the louder and more frequent the snoring, the greater the likelihood of OSA. To confirm a diagnosis of sleep apnea, a sleep study using special monitors may be conducted in a clinical setting, or an at-home test may be used.
4) Your dentist may be able to help diagnose and treat sleep apnea
What does dentistry have to do with sleep apnea? For one thing, sleep apnea is a disease that involves structures in the oral cavity — an area dentists are quite familiar with. Sometimes, fatigued folks who suffer from OSA begin snoring when they recline in the dental chair, showing their symptoms firsthand. But even if their patients don't fall asleep, dentists with proper training are recognized by the American Academy of Sleep Medicine (AASM) as being able to provide first line therapy for mild to moderate sleep disorders.
5) An oral appliance is a good step to try before more invasive treatments
If it's appropriate in your situation, your dentist can custom-fabricate an oral appliance that may alleviate sleep-related breathing disorders. This device, worn while you're sleeping, helps to maintain an open airway in the throat and to reduce breathing problems. With a success rate of around 80%, in many cases it's comparable to the more complex CPAP (continuous positive air pressure) machines, but people often find it easier to wear. Plus, it's a non-invasive treatment that can be explored before deciding on a more involved treatment, such as surgery.

A: Obstructive sleep apnea is a type of sleep-related breathing disorder (SRBD) in which the airflow to the lungs is restricted — or even cut off completely — during sleep. This condition is usually caused by the collapse of soft tissues in the back of the throat, and is potentially deadly. Sleep disorders, including SRBD, are thought to affect tens of millions of people in the United States. They have been blamed for several catastrophic accidents, including the 2014 Metro-North train crash in New York, and the 1989 Exxon Valdez oil spill in Alaska.
Q: How can I tell if I might have sleep apnea?
A: Everyone has trouble sleeping sometimes. But if you constantly snore, wake up feeling irritable, and experience sleepiness and diminished performance during the day, it may mean you suffer from this condition. After a while, SRBDs can trigger depression, confusion, memory loss, and other personality changes. Medical professionals note that a person with SRBD tends to be obese; to show enlargement of the tongue, tonsils, or uvula; to have nasal polyps or congestion; and possibly, to exhibit other signs.
Q: How is sleep apnea treated?
A: There are various treatments for sleep apnea, depending on the severity of the problem and its likely cause. These include oral appliance therapy (wearing a retainer-like device in the mouth at night); orthodontic treatment and/or oral surgery; and using a CPAP (constant positive airway pressure) machine to help facilitate breathing at night. Each has advantages and disadvantages that should be discussed with a healthcare provider who has experience in the area of sleep disorders.
Q: What does all this have to do with dentistry?
A: Dentists are, of course, extremely familiar with the anatomy of the mouth. We sometimes notice signs of potential sleep problems before they become life-threatening.
